When are you available to volunteer? Don't worry, this isn't final! You will set your own schedule with the location you choose to volunteer at, but we would like a general description of your availability.
Hours Available Per Week:
Some locations require a drug screen, possibly a background check and / or training to familiarize you with their faculty and volunteer procedures.
You must also print the UH Release and Indemnification Agreement for schools or hospitals, sign the form, and return it to Dr. Jerry Evans in Room 330, Technology 2 Building, or fax to: 713-426-6417. You will be reminded several more times--it's that important.
